Financial Information

We are participating providers with Medicare and accept assignment of all Medicare claims. This means that Medicare pays 80 % of the allowed charge. The patient is directly responsible for the remaining 20% as well as the annual deductible. If you have a secondary insurance for the remaining 20% we will file that claim as a courtesy.

We participate also with Arkansas Blue Cross Blue Shield, Medicaid and many other HMO, PPO and managed care programs. It is your responsibility to obtain any referrals from your primary care physician or your insurance company if these are required. All co-payments are due at the time of your visit.

We are happy to file with your insurance carrier. However, patients are responsible for full payment of the account balance.

A payment schedule may be arranged for balances such as those for surgical procedures. Expenses for products such as glasses or contact lenses are expected to be paid in full before dispensing.
